Gain experience in Home Care Package Management
We are currently recruiting for Care Advisors to support clients through their HomeCare Packages
You will be required to visit clients residing from Bendigo to Sunbury.
Act as the key contact person and advocate for clients in facilitating the identification of their goals and development of their care plans. You will assist the Care Organisers and Program Manager in providing, mentoring and supervision to Home Care Workers and promote a harmonious team approach to the delivery of community services.
This role will include:
Undertake assessments of prospective clients and assist in making recommendations re the capacity of UA to meet their needs
To assess, monitor, reassess and review the needs of clients within a holistic framework using a restorative and wellness approach
Respect consumer's autonomy and self determination to make choices for themselves and take calculated risks
To provide information, support and advice to clients or arrange for such information, support and advice to be provided by suitably qualified people including relevant Health Practitioners queries, at the request of the person,
To monitor and maintain records of expenditure and ensure client plans do not exceed budget allocations and are within quality standards
You will have:
1 year experience minimum in Aged Care Case Management
A valid and current Australian Driver's license
A valid and current Working With Children Check
Great report writing skills
Uphold ethical behaviour, consistent with the values of honesty, fairness, responsibility and hope
Demonstrate integrity, credibility and foster open communication
Experience and knowledge of the NDIS
You will also be willing to undergo police and Working with Children Checks, as well as providing 2 recent managerial references.
