Tradewind Australia partners with service providers across the Aged Care sector to deliver quality care and support to Victorian communities. We are currently recruiting on behalf of a highly regarded not-for-profit organisation committed to delivering responsive, compassionate and person-centred care to older Australians from culturally diverse communities.
We are seeking an experienced and passionate Team Leader to lead a dedicated team of Care Partners delivering high-quality, person-centred services under the new Support at Home program.
This is an exciting opportunity for an established leader with extensive experience in Aged Care Services, particularly Home Care Packages (HCP) and a strong working knowledge of the transition to the new Support at Home (SAH) model. This expertise is essential to successfully lead a team through one of the most significant reforms in the aged care sector.
About the Role
As Team Leader, you will:
- Lead, mentor and support a team of Care Partners to deliver exceptional client outcomes.
- Oversee care planning, service coordination, client budgets and quality service delivery.
- Provide leadership in managing complex client matters, complaints, risk and continuous improvement initiatives.
- Ensure compliance with the Aged Care Quality Standards and Support at Home program requirements.
- Foster strong relationships with clients, families, healthcare professionals and community stakeholders.
- Drive a positive team culture while contributing to organisational growth and service excellence.
About You
To be successful, you will have:
- A qualification in Nursing, Social Work, Allied Health, Case Management, Management or a related discipline.
- Demonstrated leadership experience within Aged Care Services, with a strong background in Home Care Packages.
- Mandatory: Comprehensive knowledge of the Home Care Package program and the transition to the new Support at Home (SAH) model.
- Experience in care planning, case management, client budgeting and leading high-performing teams.
- Sound understanding of the Aged Care Quality Standards and legislative requirements.
- Excellent communication, stakeholder engagement, organisational and problem-solving skills.
- A genuine commitment to delivering exceptional care and supporting older Australians to remain living independently at home.
- Bilingual candidates, particularly in Greek, Italian, Arabic or Macedonian, are highly regarded to support our culturally diverse client base.
