Job Description:
The Case Coordinator for the National Immigration Support Service (NISS) is responsible for delivering responsive and high-quality case coordination services to clients. The role involves creating and implementing individualised case plans that address client needs, promote independence, and ensure their ongoing safety and wellbeing. The coordinator applies evidence-based models such as CARE and TCI, and ensures accurate documentation of case activities. Strong collaboration with internal and external stakeholders is essential to achieving positive client outcomes.

Key Duties:

  • Develop and implement case plans based on client assessments.
  • Provide culturally appropriate support to clients from diverse backgrounds.
  • Ensure timely and accurate completion of incident reports, risk assessments, and safety plans.
  • Refer clients to appropriate support services and maintain up-to-date records.
  • Collaborate with internal teams and external stakeholders to meet case plan objectives.
  • Monitor and review client progress, adjusting case plans as needed.
  • Promote a safe and respectful environment for clients, especially children.

Skills and Experience:

  • Strong experience in case coordination within the Human Services sector.
  • Demonstrated ability to conduct needs assessments and implement care plans.
  • Proven experience working with individuals from culturally and linguistically diverse (CALD) backgrounds.
  • Ability to manage clients with complex physical and mental health issues.
  • Knowledge of relevant legislative and regulatory frameworks.
  • Excellent written and verbal communication skills, with strong analytical abilities.
  • Ability to work autonomously and in multi-disciplinary teams, managing competing priorities.
