Tradewind Australia partners with service providers across the Aged Care sector to offer quality care to Victorian communities.

We are seeking a dedicated and experienced Care Advisor (HCP) for a full-time locum role based in Melbourne's inner eastern suburbs. This assignment is expected to run for approximately six weeks, with the possibility of further opportunities within the organisation or elsewhere through our broader network. It's an excellent fit for someone who enjoys the variety and flexibility of locum work.

About the Role:

As a Care Advisor, you'll play a pivotal role in supporting consumers receiving Home Care Packages (HCP), ensuring services are delivered in alignment with their goals, preferences and evolving needs. This role suits someone who can hit the ground running and quickly adapt to new environments. You'll need to be flexible in your approach and responsive to the unique systems and practices of each organisation. Your expertise in HCP Case Management will be essential in helping clients navigate the aged care system, especially with the upcoming Support at Home (SAH) reform.

Key Responsibilities:

  • Provide person-centred case management to HCP recipients
  • Conduct assessments, develop care plans, and coordinate service delivery
  • Educate and support clients and families regarding the SAH reform changes
  • Maintain accurate documentation and ensure compliance with Aged Care Quality Standards

About You:

  • Proven experience in HCP case management is essential
  • Strong knowledge of the Support at Home (SAH) reform and its implications
  • Able to hit the ground running with minimal supervision
  • Flexible, adaptable and able to work within different organisational frameworks and systems
  • Relevant qualifications in nursing, social work, allied health or aged care
  • Excellent communication and problem-solving skills
  • Enjoys the dynamic nature of short-term assignments and new challenges
