Disability Case Manager (Support Coordinator)

About the Role
As a Support Coordinator, you'll:
Help participants understand and implement their NDIS plans
Connect people to the right mix of supports - formal, informal and community-based
Coordinate services, track progress and assist with reviews
Support participants to build capacity, independence and informed decision-making
Monitor budgets and maintain accurate records
You'll also work collaboratively with teams and stakeholders to strengthen outcomes and improve support coordination practices.
About You
You'll bring:
Knowledge of the NDIS and experience working with people with disability or complex needs
Strong communication and organisational skills
Confidence in liaising with providers and coordinating service agreements
A person-centred approach grounded in empathy, cultural awareness, and accountability
What You'll Need
Relevant qualification in community services, disability or related field (desirable)
NDIS Worker Screening Check & Working with Children Check
Current Driver's Licence and access to a vehicle
Completion (or willingness to complete) required NDIS and safety training
