Job Description

Tradewind Australia works across the Aged Care sector in partnership with providers of services to the Victorian Communities. We recruit for a range of disciplines on behalf of Not for Profit, private and government organisations.

We currently have an opportunity for Community Registered Nurse to work in an office based environment, in an administrative capacity, utilising their clinical knowledge and skillset, to oversee the delivery of services to the highest quality of standards.

Key Responsibilities:

  • Assist in the coordination of DVA Community program and general nursing referrals, monitor new referrals and on-board new clients
  • Provide assessments, support and care planning to eligible clients across the DVA and TCP nursing programs
  • Provide clinical oversight to ensure the allocation and monitoring of resources is delivered to a high level of service and in accordance with care plans and quality standards
  • Develop collaborative relationships across the health service and community programs to provide high quality and response service
  • Work in collaboration with the Client Care Coordination team and HCP Case Managers to ensure compliance with Commonwealth guidelines and ensure information e.g. Charter of Aged Care Rights are provided to all consumers.
  • Prepare high quality reports and other documentation and ensure they are submitted in a timely way to relevant stakeholders.

Prerequisites required for the role:

  • AHPRA Registered Nurse Division 1
  • Experience with MAC Portal
  • Experience in undertaking complex assessment and care coordination of clients
  • A good understanding and knowledge of the broad range of social and health issues to assist clients with service access.
  • Excellent communication skills
  • Demonstrated ability to develop, maintain and appropriately conduct professional relationships with consumers and key service providers
  • Demonstrated competency with appropriate client management systems and database applications that maintain client records and collection of statistical information

This is a 3-month locum role, to start immediately. Hybrid work conditions available.
