Early Intervention Case Manager

Support individuals to keep their homes before crisis hits

Full-Time | Mon-Fri | Business Hours


Are you passionate about early intervention and preventing homelessness before it begins? We're looking for a confident, compassionate, and community-minded Case Manager for vital early intervention service that supports individuals and families at risk of losing their tenancy in either community housing or the private rental market.

About the role:

  • Work outreach-style-you'll meet people where they're at, in the community or at home, building trust and rapport over time
  • Manage a caseload supporting your clients stabilise and sustain their housing
  • Complete strengths-based risk and needs assessments, and develop personalised support plans
  • Advocate for tenants, connect them to specialist services, and work collaboratively to remove barriers to housing stability
  • Help clients maintain tenancy agreements, meet landlord obligations, and prevent eviction

About you:

  • Minimum Cert III in Community Services (or related field)
  • Demonstrated experience in case management or outreach roles, particularly with at-risk individuals
  • High emotional intelligence, professionalism, and the ability to work both independently and in a team
  • Confidence working in public spaces, community housing settings, and private residences
  • Current Driver's Licence and valid Blue Card (Working with Children Check)
  • Passion for social justice and a commitment to dignity, empathy, and empowerment
