We're partnering with a highly respected, family-focused palliative care service in Shepparton to find a Family Support Practitioner who's ready to walk alongside children, young people, and families through one of life's most challenging journeys.
This role offers the opportunity to provide therapeutic and emotional support from diagnosis through to bereavement, helping families navigate complex emotions with dignity, respect, and connection.
๐ผ Your Impact
As a Family Support Practitioner, you will:
Deliver counselling and therapeutic support to children, young people and families
Strengthen community and social connections for families
Coordinate volunteers assisting with family and carer support
Collaborate with a multidisciplinary team to ensure holistic, wraparound care
โ What You'll Bring
Diploma in Community Services or a related qualification (or above)
1-2+ years of experience supporting children, young people or families in a therapeutic, casework or support setting
Strong communication, relationship-building, and organisational skills
A compassionate, strengths-based approach to family support
A genuine commitment to person-centred, trauma-informed care
