Tradewind Australia works across the Aged Care sector in partnership with providers of services to the Australian Communities. We recruit for a range of disciplines on behalf of Not for Profit, private and government organisations.
We are presently working with an organisation who are dedicated to supporting the elderly and their families with a range of services.
We currently have a great opportunity for an experience Aged Care Lead in a HCP program setting.
Key Responsibilities:
- Provide an exemplary level of care and case management services to the clients in the community through strong leadership and management
- Delivering person-centered and enablement-focused case management in accordance with policies and procedures, program guidelines, the regulatory frameworks and best practice guidelines.
- Dedication to overseeing, coordinating, and managing comprehensive client-directed care and services in partnership with clients and their representatives, adhering to their goals, wishes, preferences, and promoting their well-being, independence, and overall quality of life.
- Developing positive working relationships within the local service centre and across the organization as well as with healthcare and other providers involved in the provision of care and services to clients, with the aim of facilitating improved clients' outcomes.
Prerequisites required for the role:
- AHPRA Registered Nurse Division 1
- Miniimum of 3 years of experience in case management in aged care and/or disability care
- Strong management and leadership skills
- Excellent communication skills
- Demonstrated understanding of client, family and community needs
