Case Manager - Housing Sector - SCHADS Level 5
As a Case Manager, you'll work directly with individuals and families experiencing housing instability. Your work will focus on early intervention, tenancy sustainment, and empower clients to build independence and improve wellbeing.
You will:
- Provide trauma-informed, flexible case management tailored to each individual's needs
- Develop and review strengths-based support plans and connect clients with appropriate services
- Work collaboratively with other agencies to ensure wrap-around support is accessible
- Advocate for clients navigating housing, health, and community systems
- Maintain accurate documentation and contribute to ongoing service improvement
About You
You are a values-driven professional who communicates effectively, responds to complexity with clarity and empathy, and builds trust with people from all walks of life. You bring a practical, person-first approach to your work and understand the systemic challenges faced by those experiencing or at risk of homelessness.
To be successful in this role, you will have:
- A relevant degree in human services or related field
- Experience supporting people with complex needs, particularly in housing, homelessness, mental health, or family support
- Strong understanding of trauma-informed and client-centred practice
- Ability to work independently and as part of a multidisciplinary team
- Proven skills in case planning, advocacy, referral coordination, and documentation
- Proficiency with Microsoft Office and case management systems
- Current Blue Card (Working with Children)
- Current Driver's Licence and willingness to travel for outreach and home visits
