Job Description
Role Responsibilities:
Deliver high-quality client services in line with case management models, funding requirements, and program frameworks.
Complete assessments, develop and review case plans, and manage case closures.
Provide holistic, sensitive support tailored to individual client needs to help them reach their goals.
Assist clients in accessing, navigating, and engaging with appropriate services and supports.
Build supportive relationships that promote client participation and self-determination.
Ensure clients are informed about the service's commitment to the safety and inclusion of LGBTIQA+ individuals.
Essential
Minimum 1 year of recent experience in an IAP role.
Background in the community services sector.
Proven ability to build and restore relationships with individuals experiencing vulnerabilities, including homelessness
Demonstrated capability to work effectively in an IAP capacity.
Strong understanding of theoretical frameworks and strategies for supporting clients with complex needs.
- A Nationally Coordinated Criminal History Check is required
- A Working with Children check is required
- A current and valid Driver's Licence is required
