Locum contract- Immediate Housing Response Case Manager
ASAP start 4-6 weeks, $49.07 per hour, Southport, QLD
8:00am-4:00pm, office based, with client visits
What you'll do:
- Rapidly engage families sleeping rough; arrange short-term hotel placements
- Complete weekly visits, develop specific, time-bound plans, and secure longer-term housing
- Use SHIP for intake, case notes, and outcomes tracking
- Provide holistic support from intake to exit, including risk/needs assessments and daily living support
- Deliver high-quality advocacy and tailored interventions to stabilise housing, health, and safety
- Liaise with housing providers, health services, and community partners
Essentials:
- Current Driver's Licence (pool car available)
- Proven case management across housing/homelessness or related community services
- Trauma-informed, person-centred practice and strong stakeholder coordination
- Minimum cert III in community services or equivalent.
- Blue card and current police check
