Job Description

POSITION SUMMARY

The Mental Health Clinician - 26yrs+, will work within the multidisciplinary team undertaking a full range of clinical duties and supporting consumers during their patient journey in acute episodes within recovery and treatment. The clinician will utilise skill sets within their scope of practice and use these in supporting consumers, carers and supporters of mental health with the area mental health service.

Clinicians will be pivotal members of the multidisciplinary team providing a range of clinical and therapeutic interventions, including the provision of recovery-orientated practice.

A clinician will work within a 7-day roster cycle roster including afternoon shifts. The position reports to the 26yrs+ Clinical Lead (clinical matters) and on-site Team Manager (operational matters).

DETAILS OF ROLE:

The key accountabilities of the Mental Health Clinician - 26yrs+, include, but are not limited to: Clinical care

* Conduct assessments in a timely manner as determined by risk, urgency, distress, dysfunction and disability per the National Mental Health Triage Scale, utilising the CMI (Client Management Interface) database

* Perform; urgent acute clinical assessment and routine assessment, working with consumers and carers in safety planning and promoting and delivering highquality, evidence-based holistic treatment (pharmacological and therapeutic intervention) within a trauma-informed and culturally sensitive framework

* Work within a recovery-based framework

* Completion of all intake and assessment documents and where services continue complete recovery documents in collaboration with consumer's carers, significant others and the multi-disciplinary team

* Use current evidence-based practice and interventions

* Collaborate with community organisations to meet identified needs

* Monitor the consumer's physical/medical state as appropriate and indicated, promoting associated treatments in conjunction with their mental state, and associated treatments

* Provide pharmacological/psycho-education specific to the consumer's identified needs

* Provide direct clinical work including individual, group or family work

* Facilitate access to services relevant to the consumer's needs

* Undertake regular reviews and discharge planning in collaboration with the consumer, carers, significant others and multi-disciplinary team

* Adhere to responsible rostering and leave management practices

* Adhere to organisational documentation guidelines when recording treatment and support in a comprehensive, factual and sequential manner

* Participate in clinical supervision lrh.com.au Information Management

* Ensure data is collected to assist with decision making and monitoring, including Health Service Agreement targets and KPI reports

* Use the clinical record set up to allow assessment and care planning and delivery to be tracked, monitored and evaluated * Ensure integrity of data collection

* Ensure consumer medical records are utilised in accordance with legislative requirements and LRH policies and procedures

* Undertake other duties required as directed by your supervisor or management, provided these are within your competency or training

KEY SELECTION CRITERIA (Qualifications\Registrations Essential):

* Nursing: Registered Nurse with post basic qualification in Mental health Nursing, holding current registration certificate with Australian Health Practitioner Regulation Agency (AHPRA), or

* Social Work: Approved degree in social work or other qualification approved for membership of the Australian Association of Social Workers, or

* Psychology: Registration with AHPRA, or

* Occupational Therapy: Registration with AHPRA

* A current Victorian Driver's License lrh.com.au Experience\Knowledge Essential:

* Minimum 2 years post registration experience in Mental Health field.
