About the role:

This position will provide place based and outreach case management support to women and women with children who have experienced and/ or are currently experiencing family violence. The role also encompasses the delivery of intake, assessment and crisis response.

The role will be in instrumental in delivering high quality, safe and effective family violence and child well-being responses to those seeking support, safety, and recovery.

The Family Violence Services works with service users from a feminist perspective, aiming to empower and increase the safety of women and children.

Role Responsibilities:

  • Effective relationships are established with clients that encourage self-determination and participation and uphold the principles of the National Family Violence Model of Care.
  • Case management support is provided to clients in accordance with TSA case management models and standards of practice
  • Ongoing assessment is made of the client's issues, needs and circumstances from initial intake to exit
  • Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients.
  • Case plans are implemented and actively led by the participant
  • Clients are assisted to access holistic support services and establish community connections, and where needed are assisted through advocacy with service providers
  • Progress and outcomes of individual case plans are regularly monitored and reviewed and where needed action is taken to ensure the plan continues to meet client needs or goals
  • Accurate, up to date client records and data is maintained for all clients.
  • Service gaps are identified and recommendations for service delivery improvements are made to the Team Leader
  • Key performance indicators are met in accordance with outcome measurement timeframes
  • Participation in an on-call roster

Qualifications and Experience:

  • A degree qualification in social work, welfare (social sciences), community development or related field is essential.
  • Experience working in the family violence sector is desirable.
  • Experience in specialist family violence and/ or child wellbeing work is highly desirable
  • Demonstrated case management experience and skills
  • Highly developed time management and work prioritisation skills with an ability to work autonomously with minimal supervision required
  • Advance computer skills including experience in data and record management

Requirments of the role:

  • Relevant child protection requirements are obtained for example Working with children check.
  • A national police record check is required
  • Evidence of qualifications
  • Current QLD drivers licence
  • Compulsory Code of Conduct, WHS and Bullying and Harassment training modules are completed and up to date.
