Clients are funded through the NDIS and present with an intellectual or physical disability or psychosocial diagnosis. Client groups include early childhood, children, teenagers, adults.
The role:
We are seeking an allied health professional to join a large multidisciplinary team to support clients. In the role you would;
- Build and maintain strong relationships with clients and key stake holders
- Gather information using conversations, observations, assessments & intervention
- Develop effective person-centred goals with the person and their circle of support
- Empower and build the capacity of people, families and communities
- Identify, plan and negotiate on-going therapy services
- Evaluate progress and outcomes using relevant outcome measures
- Prepare professional reports and keep accurate time records
Requirements:
The successful applicant will be able to provide;
- Bachelor in Speech Pathology / Occupational Therapy / Physiotherapy
- Registration with AHPRA or SPA
- Experience working with people presenting with a disability / mental health issue
- Willingness to undergo relevant compliance checks (police, WWCC)
- Victorian or NSW Driving licence
