We are currently recruiting for Care Advisors and Case Managers, based around Forest Hill, Kingsville, Melbourne CBD And Preston

You will be responsible for providing structured support to Aged Care support services, by engaging clients and staff in a way that promotes their strengths and delivers results. You will support them in undertaking assessments, engage with other support services and implement, evaluate, and document the planning of the clients care.

Key duties:

The Care Advisor will act as the key contact person and advocate for clients in facilitating the identification of their goals and development of their care plans. Working within a multidisciplinary team, to develop the goal directed plan in conjunction with the client and to respond to client's individual needs and preferences within budget. The provision of creative options and solutions that deliver exceptional client outcomes to ensure that the delivery of services meet all associated funding agreements, professional and legal standards.

To assist the Rostering Coordinators and Program Manager in providing, mentoring and supervision to Home Care Workers and promote a harmonious team approach to the delivery of community services. Promote and participate in quality activities, and provide outstanding customer service maintaining friendly, supportive and professional relationships with clients, their relatives and friends. Support colleagues by sharing knowledge and expertise

You will have:

  • A qualification related to Administration, Aged Care or Social Work.
  • A reasonable understanding of case management framework, including assessment and intake processes
  • An understanding of the Aged Care Service System
  • Demonstrated ability to work autonomously and as part of a team.
  • Excellent written and verbal communication, problem solving skills, and negotiation skills
  • Victorian driver's license

You will also be willing to undergo police and Working with Children Checks, as well as providing 2 recent managerial references.
