Social & Community Case Managers

We are currently looking for experienced Mental Health, Homelessness, Youth & Justice workers with experience in support work, residential services, case management or clinician work based in Sydney for exciting new opportunities.
You will be responsible for providing structured support to those in the programs, by engaging in a way that promotes their strengths and empowers. We work to match your requirements and career goals to positions that are aligned with your chosen path.

You will:
- Have previous paid work experience in the social and community services sector
- Establish and maintain effective relationships with key support services and stakeholders
- Maintain accurate files, case notes, and databases
- Coach and empower clients to live a safe and independent life.
- Provide person-centres support, assessment, and referrals to clients
- Culturally appropriate support for clients facing specific barriers
You MUST have:
- A qualification in Social Work, or a relevant Health Science
- Strong verbal and written communication skills
- Competency in Microsoft programs and other Case Management software
- A valid and current driver's licence
- CPR/1st Aid
You will also have or be willing to undergo a Working with Children Check and a Police Check.
