Job Description
Tradewind Australia partners with aged care providers across Victoria, recruiting for not-for-profit, private and government organisations. We are currently seeking experienced Clinical Aged Care Needs Assessors to support assessment services across Victoria.
These opportunities sit within established Aged Care Assessment Services supporting older Australians to access the care and services they need. Remote working arrangements may be available for highly experienced candidates who meet all mandatory requirements and can work independently within the national assessment framework.
Key Responsibilities
Conduct Comprehensive and Home Support aged care assessments
Develop individualised support plans aligned with client goals and reablement principles
Generate referrals and manage assessments through the My Aged Care system
Work collaboratively with clients, families, carers and service providers
Maintain accurate documentation and reporting within national aged care systems
Participate in service improvement initiatives to support quality outcomes
Travel to client homes where required to complete assessments (depending on location)
About You
To be successful in this role you will have:
Current AHPRA registration as a Registered Nurse or Allied Health professional
Previous experience working as an Aged Care Assessor
MANDATORY: Fully signed off as a Comprehensive Clinical Assessor under the My Aged Care Single Assessment System
Completion of all required training through My Aged Care and the Department of Health for clinical assessors
Ability to commence the role and work independently with minimal supervision - candidates must be able to hit the ground running
Experience working with older people in acute, rehabilitation, community or aged care settings
Strong clinical assessment, care planning and communication skills
