Locum - Expression of Interest
Locations: Across Metro Melbourne
We are seeking experienced Clinical Care Advisors / Case Managers to register their interest for locum opportunities across Home Care Packages (HCP) and Support at Home (SAH) programs throughout Metro Melbourne.
These opportunities are ideal for clinicians who enjoy flexibility, variety, and making a meaningful impact supporting older Australians to live safely and independently at home.
About the Role
As a Clinical Care Advisor / Case Manager, you will play a key role in coordinating and overseeing client care, ensuring services are clinically appropriate, person-centred, and aligned with funding and compliance requirements.
Depending on the assignment, responsibilities may include:
- Managing a caseload of HCP and/or SAH clients
- Conducting comprehensive client assessments and care planning
- Coordinating services with internal teams and external providers
- Monitoring clinical outcomes, risks, and service delivery
- Supporting compliance with Aged Care Quality Standards and funding guidelines
- Building strong relationships with clients, families, and stakeholders
About You
To be successful in locum assignments, you will bring:
- Clinical background (RN, Allied Health, or equivalent experience)
- Previous experience in Home Care Packages and/or SAH
- Strong care coordination and case management skills
- Sound knowledge of aged care funding, compliance, and clinical governance
- Excellent communication and documentation skills
- Ability to adapt quickly and work autonomously in new environments
