Tradewind Australia partners with service providers across the Aged Care sector to offer quality care to Victorian communities. We are currently working with a respected not-for-profit organisation dedicated to providing responsive, compassionate and high-quality care to local communities.
We are seeking an experienced Registered Nurse with a background in Care Advisory roles within the Home Care Packages (HCP) program to join this passionate and committed team.
Key Responsibilities:
- Comprehensive Assessments & Care Planning:
Conduct clinical assessments and develop tailored care plans based on the clients' medical, personal needs, goals and preferences. - Clinical Oversight & Service Coordination:
Ensure the safe, high-quality delivery of care by overseeing clinical services and coordinating with a multidisciplinary team. - Monitoring & Adjusting Care:
Regularly review and adjust care plans to accommodate any changes in the client's health status or care needs, ensuring optimal outcomes. - Collaboration with Healthcare Providers:
Work alongside GPs, allied health professionals, and other service providers to deliver coordinated, person-centred care addressing both clinical and non-clinical needs. - Compliance & Documentation:
Maintain accurate, up-to-date clinical documentation and ensure compliance with Home Care Package guidelines and regulatory standards. - Education & Support:
Provide clinical guidance and support to care staff, clients and families to ensure safe and effective care delivery.
Key Requirements:
- Bachelor of Nursing and current AHPRA registration
- Proven experience in a similar role within the Aged Care sector
- Extensive knowledge of Home Care Packages (HCP), aged care support systems, and funding mechanisms
- Strong understanding of the challenges faced by older people, their families, and carers
- Experience delivering person-centred care and supporting models that promote client empowerment
- Excellent interpersonal skills with the ability to work in a multidisciplinary team
