Family Wellbeing Case Manager
Pay: $75k -$103k + super + salary packaging (experience-based)

Tradewind Australia is partnering with a well-known not-for-profit community service to recruit an experienced and culturally responsive Family Wellbeing Case Manager. This full-time role offers meaningful, community-based work supporting Aboriginal and Torres Strait Islander families experiencing vulnerability, family stress, and child safety concerns.
This role is suited to practitioners who value relationship-led, trauma-informed and culturally safe practice, and who are comfortable working with families in complex, high-need contexts.
About the Role
As a Family Wellbeing Case Manager, you will deliver holistic, culturally safe case management to families experiencing vulnerability, including housing instability, domestic and family violence, mental health challenges, and child protection involvement.
Working closely with families and kinship networks, you will support safety, wellbeing and family capacity through coordinated service delivery, strengths-based engagement and practical support. The role is community-facing and involves home-based outreach, family meetings, and close collaboration with health, child protection, youth justice and community services.
You will play a key role in strengthening family safety, supporting reunification where appropriate, and improving long-term outcomes for children and young people.

Key Responsibilities
- Engage families through outreach and home-based visits using motivational and strengths-based approaches
- Complete culturally safe assessments that centre client voice and family perspectives
- Facilitate family-led decision-making processes with kinship networks
- Deliver case-managed support aligned with the program model of care
- Undertake safety assessments and respond to DFV, mental health and child safety concerns
- Coordinate wrap-around supports across health, housing, child protection and community services
- Maintain high-quality case notes and client documentation in line with legislative and organisational requirements
What You'll Bring
Essential
- Degree in social work, human services, psychology, community services or related field (or equivalent experience)
- Strong experience in family support, child protection, youth, housing or community-based case management
- Demonstrated trauma-informed, culturally safe and family-centred practice
- High-level communication, engagement and documentation skills
- Ability to work autonomously in the community and collaboratively within a multidisciplinary team
- Current driver's licence and ability to travel across Brisbane
Desirable
- Experience working within Aboriginal Community Controlled Organisations or culturally specific service settings
- Experience facilitating family-led decision-making or kinship-based interventions
