Job Title: Home Care Package Case Manager (Casual Contract)
Location: Mascot, NSW
Position Type: Casual - 4 Week Assignment (Potential to Go Permanent)

About the Role

We're seeking an experienced aged care professional for a 4-week casual assignment based in Mascot, with the potential to move into a permanent role.

This is a non-clinical, community-based position supporting older people to remain living independently at home. You'll manage a caseload of Home Care Package clients, coordinate services, oversee budgets, and ensure supports are aligned with client goals and funding requirements.

Please note: this is a casual role, and applicants must be comfortable with the short-term nature of the assignment, with potential for extension or permanency.

Key Responsibilities

  • Manage a caseload of Home Care Package (HCP) clients

  • Conduct assessments and develop tailored support plans

  • Coordinate services and liaise with providers, clients, and families

  • Monitor and review plans to ensure outcomes are being achieved

  • Manage package budgets and maintain accurate documentation

  • Ensure compliance with Aged Care Standards and funding guidelines

  • Travel to clients' homes as required

Essential Requirements

  • Previous experience working in aged care (community-based)

  • Demonstrated understanding of Supported at Home / Home Care Packages (HCP)

  • Strong working knowledge of CHSP and HCP funding models

  • Previous experience in a Case Manager or Intake role within aged care

  • Strong administrative, organisational, and time management skills

  • Ability to manage budgets and compliance requirements

  • Comfortable working autonomously and in a fast-paced environment

  • Current driver's licence and willingness to travel locally

This role will suit someone who can step in confidently, manage their own caseload with minimal supervision, and understands the operational and funding frameworks of community aged care.
