Client Outcome Facilitator - Women & Children
Contract: 4 weeks from 20th October
Hours: Mon-Fri, 8:30am-4:30pm (no public holidays)
$49 per hour (casual rate)

The role:
- Provide holistic, end-to-end case management, advocacy and practical support to clients engaged through the program, with a focus on client needs and building capacity toward independence.
- Enable clients to reconnect with community by providing information, referrals and warm handovers within approved case management plans.

Qualifications & experience
- Minimum Certificate III in Community Services (or related field)
- Well-developed interpersonal, communication and organisational skills; sound judgement; proven ability to contribute effectively in a team environment.
- Demonstrated ability to advocate on behalf of clients and maintain confidentiality of client information at all times.
- Current Blue Card and Police Check
