Tradewind Australia partners with aged care and disability providers across Victoria, recruiting for not-for-profit, private and government organisations. We are currently working with a well-established not-for-profit that supports older people and their families.
An Aboriginal Community Controlled Health Organisation in southern Melbourne is seeking an experienced Registered Nurse for an 8-week locum assignment based in Cranbourne West.
This role delivers client-centred clinical assessments and care planning for younger people and eligible Aboriginal and Torres Strait Islander clients under the CHSP & HACC PYP programs, supporting independence, reablement, and safe community living.
Key responsibilities include:
- Comprehensive health assessments, care planning, and referrals
- Chronic disease management, wound care, continence, medication and complex care
- Collaboration with multidisciplinary teams, carers and community services
- Accurate clinical documentation and compliance with program guidelines
- Culturally safe, trauma-informed and strengths-based practice
About you:
- AHPRA registered Registered Nurse
- Minimum 2 years' community, disability or complex care experience
- Strong understanding of reablement and community nursing models
- Excellent communication and documentation skills
- Driver's licence, police check, and up-to-date immunisations
- Experience working with Aboriginal and Torres Strait Islander communities highly regarded
