Are you a dedicated Social Worker with support coordination experience?
Join us in making a meaningful difference in the lives of NDIS participants by providing exemplary support and coordination service delivery to all participants under their NDIS package across Brisbane, Sunshine Coast, Ipswich and Gold Coast.
$80-90k plus company car, mobile, and laptop.
Responsibilities:
- Assist participants in understanding and utilising their NDIS plans to achieve goals, and live independently, increase skills, and engage with their community and employment.
- Connect participants to allied health supports (OTs, Physiologists, Psychologists, Nurses, Dietitians), link to community, mainstream, and government services.
- Monitor, guide and assist participants exercise choice and control and coordinate supports and reviews.
- Complete necessary forms and risk assessments per policy and procedure manual and code of conduct.
- Contribute to building a stakeholder network
- Participate in meetings and share relevant training opportunities
Required:
2 years of experience in NDIS support coordination
Bachelor of Social Work or equivalent
Registration with AASW or ACA or recognised body
Experience in aged care / home care service/support desirable
