Job Description
An exciting opportunity is available for an experienced Case Manager to join a multidisciplinary team within the MyLife program, supporting young people through Targeted Care Packages. This program focuses on early intervention and transition support, helping young people either avoid entering, or successfully move out of, residential care.
About the Role
As a Case Manager, you will play a key role in delivering individualised, trauma-informed support to young people with complex needs. You will work collaboratively with youth workers, families, and external stakeholders to ensure coordinated, holistic care that reflects the goals and strengths of each young person.
Key Responsibilities
- Manage end-to-end case management in line with statutory requirements
- Develop and implement individualised care plans tailored to client needs
- Work in partnership with young people, families, carers, and multidisciplinary teams
- Provide guidance and support to youth workers
- Advocate for clients and support access to appropriate services
- Contribute to assessments, reporting, and data collection
- Support young people to build skills, independence, and long-term stability

Key Requirements
- Must have a relevant Bachelor Qualification
- Minimum 2 years of paid relevant experience
- Demonstrated experience in case management within a statutory or child and family services setting
- Strong understanding of:
- Trauma, attachment, and child development
- Out-of-home care and the broader service system
- Ability to manage competing priorities and work effectively under pressure
- Excellent communication, stakeholder engagement, and teamwork skills
- High level of IT literacy and documentation skills
