Youth Housing Case Manager

Pay: SCHADS Level 5 +casual loading = $63.76 per hour + superannuation
Location: Beenleigh , QLD
Employment Type: Full time - Fixed Term (March to June)

Tradewind Australia is partnering with a respected community youth housing and homelessness service in the Logan region to recruit two full-time Youth Housing Case Managers for a short-term contract from March to June. This role supports young people aged 16-25 who are experiencing or at risk of homelessness, helping them build independence, stability and connection to community supports.

This is a hands-on, youth-focused role where you will work directly with young people to strengthen life skills, support housing outcomes and coordinate access to services that promote long-term wellbeing.


About the Role

As a Youth Housing Case Manager, you will provide intensive, person-centred case management and practical support to young people experiencing housing instability or homelessness. Working within a multidisciplinary team, you will support young people to identify goals, navigate services and build the skills needed to maintain safe and stable housing.

You will also collaborate with community partners, housing services and support networks to ensure coordinated, trauma-informed support.

    Key Responsibilities

  • Provide trauma-informed case management to young people aged 16-25
  • Conduct intake, assessment and support planning with young people
  • Assist clients to access housing, education, employment and community supports
  • Provide advocacy, information and referrals to relevant services
  • Support young people to build life skills, independence and stability
  • Maintain accurate case notes, documentation and client records
  • Collaborate with internal teams and external stakeholders to coordinate care
  • Participate in team meetings, case reviews and program activities


    What You'll Bring

    Essential

  • Degree qualification in Human Services, Social Work, Youth Work or a related field
  • Experience supporting young people experiencing or at risk of homelessness
  • Demonstrated case management, assessment and advocacy skills
  • Strong ability to engage young people from diverse backgrounds, including those experiencing complex needs such as mental health challenges, family violence, financial stress or substance use
  • Strong communication, relationship-building and documentation skills
  • Ability to work independently while contributing to a collaborative team
  • Current Driver's Licence
  • Current Blue Card (Working with Children Check) or willingness to obtain

    Desirable

  • Experience working in youth homelessness or housing services
  • Knowledge of trauma-informed and strengths-based practice
  • Experience working with Aboriginal and Torres Strait Islander and culturally diverse young people
